Today I went into my pediatrician for my weight check. When I got my Synagis shot last week I was 9 lbs 1 oz. This was in the midst of all of my spitting up/vomiting problems. Today I weighed in at 9 lbs 4 ozs which is right on track for the 3rd percentile for weight. I have been in the third percentile from the beginning so my doctor was very happy.

Mommy had lots of questions for the doctor about my spitting up. Normally, spitting up once a day or once a week wouldn't be a cause for concern. Due to my trach, though, it can create other issues. Since I have been doing so well over the last few days, my doctor doesn't want to put me on a medication for acid reflux - although I have all the classic signs. Instead, she said just switching to formula may help now because it is a little heavier then breast milk. If Mommy wants to experiment she can try adding some rice cereal to my bottles or starting me on formula that already includes some extra starch that is made for babies who spit up all the time. Mommy is going to give me a week on just the formula to see what happens since I have not been spitting up much lately then decide if she wants to try out some other options.

My Mommy asked about my ears since I like to pull on my left ear so much. That is the ear where the canal is closed off. The pediatrician said that ear pulling is not necessarily a sign of an ear infection but she is kind of concerned how she would ever diagnose an ear infection in that ear since the canal is closed off. She wants Mommy to follow up with the audiologist to ask that question. She also suggested giving me Tylenol before we get on the plane so if I do have any problems with my ears it might help with the pain.

When I got weighed Mommy had to strip me down until I was naked. It took a few minutes for the scale to become available so I had some time to be nude and I loved it! I kicked my legs and smiled big smiles for Mommy and the nurses. Luckily, I didn't get so excited I peed (my brother Chili does that) so Mommy thinks I should love getting in the bath once she gets the okay from the pediatric surgeon that my button site is healed. All in all, it was a good day at the pediatrician's office - no shots!

Next week will be really busy for me. I have a follow up appointment on Monday with the pediatric surgeon to look at my g tube. On Wednesday I have a four hour appointment with the craniofacal panel to discuss my plan of care. On Thursday, I have my audiology appointment to have my hearing tested. That appointment could take all day if I don't fall asleep and stay asleep long enough! Mommy is a little stressed at the thought of all of those appointments in one week but I will try to be a good girl for her and not spit up or cry too much.

Another good day at the Smith household - at least when it comes to my spitting up. I had another spit free day today and that is with me getting 100% formula now.

I was in a strange mood today. I didn't want to hang out in my crib at all so Lucille ended up holding and rocking me instead. I didn't have any spit up today, though which made Mommy really happy. My 5 pm feeding was my last breast milk feeding. Starting tomorrow I will be completely transitioned to formula. I think Mommy was a little sad but it was okay by me.
Since I have started on formula I have a lot more gas and it comes out either end. In the last week Mommy has started hearing me burp for the first time. She wasn't sure that is what it was the first couple of times since burping through a trach sounds really strange.
Mommy took some pictures of all my machines that we will be bringing with us to Salt Lake. The first is my pulse ox machine that monitors my oxygen levels and my heart rate. This is the most irritating machine to my parents because it alarms if I move around too much or my lead isn't attached well. The second is my trach mist machine that gives me warmed air since with my trach I can't humidify the oxygen I breathe. The third is my Kangaroo pump that pumps my feedings into through my Mic-KEY button. The last machine - and probably the most important - is my suction machine. That is how my parents and nurses get my secretions out of my trach and suction up my spit ups. Saturday night they had a scare again when the lid on my suction machine cracked and they couldn't get a good suction. They called my DME company and told them they would have to send someone out with a new lid. When the rep showed up he explained that the lid design was changed several months ago and many clients are having an issue with it cracking after several weeks. Since this was the second time it has happened to my parents felt a little better that it wasn't something they were doing wrong. They are trying to get the insurance to authorize more lids a month so they don't have to worry about this in the future. So if you see me in Salt Lake you might see a few of my machines too and now you'll know what they do.

Can you believe it - I am three months old today! Look at how much I have grown! My days at the NICU seen like a faint memory. I have now spent more time home with my parents then I spent in the NICU - what a milestone. I had a pretty good day for my three-month birthday. I did spit up after my 5 pm feeding while Daddy was cutting my fingernails but I was still a happy girl most of the night.

I don't want to jinx myself, but it has been two days now (Sunday & Monday) without any spitting up for me. Mommy is so excited - espescially since she introduced a formula feeding during the day. As long as I am spit free, she is going to introduce another formula feeding during the day until I am getting all my feedings that way. None too soon, either, she is just not producing much breast milk anymore. She is going to keep pumping though for a few weeks and build up a little supply to use with my Haberman feeder since that is what I am used to getting. At some point she will introduce me to formula orally as well so I can get used to it.

I had a really good night with my parents. Mommy was blowing on my belly and got me to laugh a few times. She thinks I am just the cutest when I am laughing. For the most part I am a pretty happy girl. I only get fussy when I am getting tired or when I have tummy troubles. Mommy, Daddy, the boys and I spent a little time together on the couch watching TV tonight. I like to watch the lights on the TV change - it will keep me entertained for quite awhile. Mommy and Daddy want me to spend more "pack time" with the family - and away from my crib. I love seeing the house from different angles and places - the couch, the changing table, the rocking chair. I definately expect to be entertained now. I am getting to be such a big girl now.

Thursday, November 22nd - Sunday, November 25th

It has been a long holiday weekend for my parents. I am still having spit up issues and Saturday started to feel like a repeat of last Saturday when I was throwing up. I had a nice Thanksgiving with my parents. Daddy made a great meal for Mommy - prime rib, green beans, grilled artichoke and pumpkin cheescake. He is a really good cook!

In the midst of having problems with spitting up, Mommy had to switch me to formula for my continuous feed at night because she is not producing enough breast milk anymore. Over the next week she will gradually get me off of breast milk completely since her supply is dwindling pretty quickly. As a result of the change, I seemed to have more issues over the weekend with gas so Daddy and Mommy broke out the Mylicon. They are not sure yet if it made much of a difference since I spit up so much on Saturday but they'll keep trying. Mommy is going to talk to the doctor at my next appointment to see if she has other suggestions for keeping my milk in my belly - everything else Mommy has tried doesn't seem to be working too well.

Check me out in the Nager & Miller's newsletter!
Today Mommy swore I laughed for her. She was putting my binki in my mouth and I would give her a big grin and then chuckle a little. She could tell by the sound of air going in and out of my trach. I had some spitting up, though, after my 8 am feeding so Mommy had Lucille skip one of my feedings so we wouldn't have any spit up issues when I went for my Synagis shot today. I was a really good girl. I cried for just a minute and then was better.

It was very quiet for most of the day today because my furry brothers went to Camp Bow Wow for their "interview". Since Camp Bow Wow has web cams, Mommy had the ability to watch them on via her computer all day to make sure they were being good. You can see a snapshot from the web cam above where they met up with some Bichon's. Mommy says they were trolling for girls. They had a great time and played well with the other doggies. The camp counselor did say that Henry and Chili hung out a lot on top of a plastic bridge outside and when other doggies tried to get on the bridge Henry would chase them away. No matter where he is Henry is alpha! When Mommy picked them up the counselor gave her a certificate showing that Henry and Chili had passed and would be allowed back at camp whenever they want. Mommy is going to have them go for day care at least a time or two before the Christmas holiday so they have more time to get used to it. Needless to say the boys were exhausted after a day of playing and fell asleep on the couch pretty early. The way they are wrapped up in the blanket it looks like one really long doggy but it is really Henry's bum and Chili's head.
Unfortunately, I had another spitting up incident with my 5 pm feed - it was the third day in a row. It happened right in the middle of my feeding so Mommy turned off the feed and I only got half my dinner. The next hour or so I was pretty gassy so Mommy isn't sure if that is what caused the spitting up. She is just going to keep hoping that I have a spit-up free day soon and try to figure out what might be causing it. Despite that, I was still a pretty happy baby tonight. Daddy held me after he got home and brushed my hair. Daddy says I need a hair style so he was experimenting in the picture above. Mommy did not like that one too much, she thinks I look like a kid on the Munsters.

Since I spit up last night, Mommy and Daddy had held off on my trach change until tonight. Unfortunately, I spit up again tonight after my 5 pm feeding. This always stresses my Mommy out but Daddy keeps telling her that all babies spit up and not to worry.
Despite spitting up again tonight, Mommy and Daddy went ahead and changed my trach. When Daddy tried to put it back in he had trouble the first time because I was crying but a few seconds later he tried again and it went in just fine. My button site is looking much better and the granulation tissue is just about gone.
Now I know there will be lots of people who want to buy me presents for Christmas but I really don't need anything. If you insist on getting me something I would love to get books. It would be even more special if you got me your favorite book as a child and wrote a little note in the cover of the book telling me why. That way, I will think of you every time I read it (or my parents read it to me).

Saturday, November 17th & Sunday, November 18th

Mommy had big plans to take pictures of me in my Christmas dress this weekend but I had a "projectile vomiting incident" Saturday morning. I wasn't able to keep down my 8 am feeding and was not a happy girl. My parents called the on call pediatrician and he recommended waiting to feed me again for several hours and reducing the volume.

Saturday night I slept really well and was pretty happy until the end of my 5 pm feeding on Sunday when I spit up a little. For the next hour I was really grouchy. I was lying on a pillow on Daddy's lap and was really fussy. My brother, Henry, whined by the chair until Mommy picked him up and put him next to me on the pillow. I think he was worried about me! Mommy noticed it had been 12 hours since the last time I had a messy diaper so she figured that was the problem. Mommy and Daddy decided to hook up the bolus feeding extension and "vent" me to get some gas out of tummy. After doing that a few times I was perking up and Mommy got the messy diaper she had been waiting for. Life was good again after that. Since I had spit up at 6 pm and my trach change was scheduled for 8, my parents decided to hold off on my trach change another night to make sure they didn't cause any more spitting up by changing my trach.

Despite the stress of my tummy troubles this weekend, Mommy noticed a few new tricks of mine. I am using my hands a lot more and when I am sitting with my Mommy I like to hold onto her arm. Before Daddy trimmed my nails on Sunday it was less like stroking and more like scratching! I also started putting my hands together and playing with my fingers, that was kind of fun. Now when I get really excited I try to roll over onto my belly. Mommy is watching me really carefully on that to make sure that I don't roll over all the way and cover my trach hole. I haven't gotten all the way over yet but I am trying. Mommy put me in the glider my Aunt Kristin bought me the last time she came for a visit. I look so tiny!

Happy 1st Anniversary Mommy and Daddy! Yes, a year ago today, my parents got married at Churchill Manor in Napa, California. It's been a big year for them.
Mommy got Daddy a globe because she says he gave her the world when he married her. Daddy stuck to the paper theme and got Mommy paper roses and a photo mosaic of their wedding picture made up of smaller wedding pictures. It is really cool. If you have never seen them before you can check out this website: http://www.designamosaic.com/photo-mosaic-process
Daddy also brought take out from Outback along with candles and a table cloth. He said if he couldn't bring Mommy out to dinner he would bring the dinner to her! Isn't he romantic - Mommy thinks so! Mommy loved her prime rib and crab - and they made it 1/2 way through dinner before I woke up. I was up for a few hours and then fell asleep by 8:30 so Mommy and Daddy had some quiet time together before Mimi arrived. Mommy and Daddy had a nice anniversary together and are looking forward to all the anniversaries to come.

Thursday, November 15th, 2007

It was a tough morning for my mommy. She decided that my nails were getting awfully long so she was going to cut them for the first time. (Now she knows why the NICU doesn't cut fingernails). The first few went just fine and she was feeling all confident. Then, she wasn't paying as much attention as she should and she pinched my finger - rather then cutting my nail. I started crying - and so did she - although she cried a lot harder and longer then I did! She then called Daddy up and said she was a horrible Mommy and he is doing all future nail cutting!
She felt really bad, espescially since she said I don't have any extra fingers to spare ; )

Mommy cheered up later in the day when she got a package from my grandma. It had an anniversary present for her and Daddy (1 year on 11/16) and several things for me. The best was the Christmas dress & matching shoes. They are so cute, Mommy can't wait to put me in them and get some pictures so you all can see. She keeps joking with Daddy that he needs to dress up like Santa Claus so she can get her Kodak moment since she doesn't want to take me to the mall to see the real Santa and expose me to all those germs. Anyone want to dress up like Santa so Mommy can get her Kodak moment?

Look how big my belly is getting! Mommy and Daddy are amazed at how big and round it is now.

So yesterday, I slept pretty much all day. Today I was awake almost all day. I just can't settle on a routine. Lucille said even though I didn't really sleep much I was a very happy girl and content to swing or watch my mobile.

The granulation tissue around my button is almost gone, but as a result now my button doesn't fit so good. Mommy and Daddy have tried a few different things to make it a tighter fit but it will probably be loose until they go back in again the beginning of December. They figure at that time the doctor will probably want to replace it with a smaller one.

My parents started reducing the time I am getting my feeds today and have a plan through mid-December to slowly get it down to 20 minutes. I only had one spit up today for Lucille so hopefully I will continue to tolerate it well.

Mommy has been working more with me and my binki. In the hospital I would take the purple newborn one but at home I wasn't taking to it too well. My mommy had ordered the next size up (green) and tried that with me instead and I seem to like it better. She also ordered a Waubbanub with the green pacifier and I am starting to like that too - you can see a picture of it above. I can suck on my bink and hold the stuffed animal at the same time.

Tuesday, November 13th, 2007

This is one of only a few times today that I was awake. In a nine hour shift with Lucille I slept for over 7 hours of it. Then, at night with my parents I was only awake about an hour. While my Daddy was holding me I spit up - an hour and 15 minutes after the end of my feeding which is kind of weird for me. Then, I was really fussy and uncomfortable for another hour or so before finally falling asleep. In total, I was probably only awake a few hours total today. Mommy thinks I might have had a tummy ache so hopefully I will be feeling better tomorrow.

A note from my Mommy:

Sometimes, it is easy to get wrapped up in all of Peyton's medical issues. There is constant coordination with the nursing & DME companies, the various specialists caring for her and the agencies and insurance companies that pay for all of it! Last week I spent several hours on the phone with Delta to discuss how to travel with all of the machines she needs on a daily basis. Despite all of that, more and more I think of myself as just a mommy - not a mommy who has a baby with disabilities. I found this essay the other day and thought it did a good job explaining better than I could what it has been like for me. In the beginning it was so easy to focus on the "problems" that I failed to see all the wondeful things about Peyton. Now, I look at her and instead of seeing her fused finger or her trach, I see her beautiful smile and the dimple that is just like mine. There are many times I forget that she is not a "normal" baby because in so many ways she is. So for those of you who get to go to Italy send me back a post card because I'm having a wonderful time in Holland and I plan on staying awhile.

Welcome To Holland by Emily Perl Kingsley

"I am often asked to describe the experience of raising a child with a disability - to try to help people who have not shared that unique experience to understand it, to imagine how it would feel. It's like this...
When you're going to have a baby, it's like planning a fabulous vacation trip - to Italy. You buy a bunch of guide books and make wonderful plans. The Coliseum. The Michelangelo David. The gondolas in Venice. You may learn some handy phrases in Italian. It's all very exciting.
After months of eager anticipation, the day finally arrives. You pack your bags and off you go. Several hours later, the plane lands. The stewardess comes in and says, "Welcome to Holland."
"Holland?!?" you say. "What do you mean Holland?!? I signed up for Italy! I'm supposed to be in Italy. All my life I've dreamed of going to Italy."
But there's been a change in the flight plan. They've landed in Holland and there you must stay. The important thing is that they haven't taken you to a horrible disgusting, filthy place, full of pestilence, famine and disease. It's just a different place.
So you must go out and buy new guide books. And you must learn a whole new language. And you will meet a whole new group of people you would never have met.
It's just a different place. It's slower-paced than Italy, less flashy than Italy. But after you've been there for a while and you catch your breath, you look around...and you begin to notice that Holland has windmills...and Holland has tulips. Holland even has Rembrandts.
But everyone you know is busy coming and going from Italy...and they're all bragging about what a wonderful time they had there. And for the rest of your life, you will say "Yes, that's where I was supposed to go. That's what I had planned." And the pain of that will never, ever, ever, ever go away....because the loss of that dream is a very very significant loss.
But...if you spend your life mourning the fact that you didn't get to Italy, you may never be free to enjoy the very special, the very lovely things...about Holland."

Saturday, November 10th and Sunday, November 11th, 2007

My parents and I had a nice weekend together. On Saturday, I went for my first official (non physician) outing. We went to Target! I like to shop! I also got dressed up for the first time in an outfit and shoes. It's a lot easier to keep my socks on my feet when I have shoes on! Don't I look like a big girl in my car seat!

Mommy moved my feedings up again to 90 cc an hour. Now, as I am tolerating the new volume Mommy will start to reduce the time I am getting it - withint 20 to 30 minutes eventually. Once that happens there will be a larger window of time that my parents can take me out places without having to worry about my next feeding. Right now it takes me an hour to eat, then we have to wait 30-45 minutes to make sure I don't have any reflux problems, so that leaves about an hour and 15 minutes until it is time to start the process again. Not a lot of time to get much shopping in!

Since I won't be back in my parent's bedroom anytime soon, my Daddy and Mommy decided it was time to decorate the dining area with my things so it is more like my own room. Despite Mommy's protests to anything pink, everything seems to have ended up pink anyway!

We had a little excitement tonight with my feeding. Mommy has said the feeding pump has been being inconsistent - sometimes I am getting feedings too fast and sometimes too slow. She never heard back from the DME company so figured I wouldn't get a new one until next week. Then at 9:30 pm someone showed up to drop off another pump. Since my parents weren't expecting them my Daddy had already hooked me up to my continuous feeding that starts at 8 pm. It is 125 cc's (25 cc's an hour for five hours and then it is changed out for another 125 cc's). So, I had already gotten about 40 cc's when Daddy had to change out the pump. He forgot to close off the feeding tube (it's okay Daddy, I know you didn't mean to) and I got the rest of my feed (about 85 cc's) in about 30 seconds. Oops! So I was sleeping good until then but as soon as my tummy filled up that fast my eyes popped open and I was wide awake and happy as could be. Daddy was able to pull out about 20 cc's manually with a syringe but I still ended up getting a lot of milk at once. Mimi then arrived so Mommy and Daddy told her what had happened and asked her to watch me really closely for the next 30-45 minutes to make sure I was okay. She elevated me so I was sitting up to reduce the likelihood that I would spit up. Daddy was still pretty worried and stayed awake a while just to make sure I was okay, which I was. A little adventure to end the night!

Now that Mommy is back to working full time my furry brothers are back to their old routine - sleeping all day!

Mommy played with me tonight because Daddy had to work late. She pulled out the mirror my Aunt Kristin got me when she was here and I was checking myself out. You can see my big grin above as I am looking at myself - guess I like what I see!

I got lots of presents today. From my Mommy's friends Scott and Marianne I got a cute pink Christmas package with snowman booties. Mommy can't wait to put me in them! It also included a rattle/blankie and I liked it right away! My great-grandpa Gil sent me a "Pooh" outfit for when I am a little older. It is very cute - good job picking it out grandpa Gil! Then my night nurse, Mimi, brought me two cute shirts. One says "up all night, sleep all day" and the other says "my mommy is nuts about me" and has a squirrel on it.

Mommy thinks something is wrong with my feeding pump and put a call in last night to the DME company. Over the last few days it will sometimes give me my feeding way too fast or too slow. Sometimes it is just right. Since it is not to be trusted Mommy wants them to bring me another one.

It was a pretty quiet day at home for me. The nursing agency sent out a supervisor to do a 30 day evaluation and realized that a lot had changed in 30 days! They had to write out a bunch of stuff for my doctor to sign.
Mommy is back to work full time but she stops by to see me during the day in between meetings and gives me kisses when she can. We get our alone time in the morning together. I am not a morning person though, and I am grumpy for a few minutes until I am really awake (just like my Daddy!)

Guess what? I am coming to Salt Lake City for Christmas! Hooray! Mommy booked the tickets yesterday, we are scheduled to fly in on December 18th and go back home on December 25th. This will allow me to meet all my SLC families and friends and I also have an appointment with Dr. Morales on December 19th to access me for my jaw distraction surgery. See you then!

Monday, November 5th, 2007

It was another exciting day! First, Lucille started her new shift - 8:30-5:30. Now Mommy can hopefully get full time hours in again.
Today I had my dr's appointment at Shriner's Hospital with Dr. James. They are very nice at Shriner's, they will do my hand surgeries for free! I slept in the car and up until they had to take my weight at the hospital. With my clothes on I am a whopping 9 lbs now! I was all smiles when the resident, Tito checked out my hands, arms, shoulders, feet and hips. In fact, I was almost laughing. Everyone commented on my cute dress and all my hair. I know I am a pretty girl but I never get tired of hearing it!
Then Dr. James came in to give my parents an idea of what they plan on doing for me. On my right hand I have four fingers and an underdeveloped thumb. Basically, although I have a thumb it doesn't really work. Dr. James wants to remove my thumb and then take my finger that does work and shorten it and rotate it to be in the position of where my thumb would be. Then, I will have three fingers and a "thumb" on my right hand. That will give me more function on my right hand then I have now. Since my trach is a really good airway for surgery she would like to do the surgery before my trach is removed. So as soon as my Mommy and Daddy know when my jaw distraction is scheduled then she wants to fix my right hand before the jaw distraction surgery while my trach is still in place.
On my left hand she said my fused finger actually looks like it is two fingers and a thumb all together. Since I use it all the time she wants to wait until I am a year or so before doing any type of surgery. She wants to see how I am using it in the future and then make a decision on what to do to correct it. It's possible that I will have three fingers and one "thumb" on both of my hands - a matching set!
As far as my fused elbows she said that surgery to correct this is not very successful so she doesn't think it's worth doing anything about. As far as my rotation, of the two types of rotation I have the better one. She said there are only a few things I won't be able to do - hold out my hand for change and catch "grounders" when I am playing baseball. That's about it!
My shoulders don't work so well either but I think the recommendation is just to get physical therapy to try to develop my muscles more so I am not so stiff.
When it comes to my overlapping toes Dr. James said I probably can't wear high heels but then I probably shouldn't be wearing those anyway. Otherwise, they shouldn't impact me walking and they are very cute so no sense doing any type of surgery to fix them.
I was such a good girl for my appointment, even though I ended up getting my feeding late. Later in the evening I sat with Daddy while he watched TV and I fell asleep. I like to blow bubbles a lot so you can see them in the first picture. Not very lady like but awfully cute!

Satuday, October 3rd and Sunday, October 4th, 2007

My Uncle Shawn left to go back to Salt Lake today. I was sad to see him go. I hope to see him at Christmas though if the doctor's approve me to fly. Before he left, he brought back In N Out for lunch. Mommy thought it was okay but not as good as when she had it at the original store in Southern California.

Saturday was my Grandpa Russ' birthday and Sunday was Karen's birthday. I wish I could have been there to spend it with them. Happy birthday!

I had some spitting up issues after my 8 am feeding on Sunday morning. Mommy isn't sure if it was due to increasing the feeding or starting my feeding early (she thinks my night nurse forget the second half of my continuous feed the night before). Whatever it was I spit up like 4 times in 15 minutes which resulted in several outfit changes and got Mommy all flustered. In the midst of this the suction machine stopped working (not a good time to have that happen). Daddy figured out the canister lid had gotten cracked somehow so it couldn't get any suction. Once Daddy put the new one on it was fine. A little too much stress that early in the morning. The rest of the day I was fine though and had no more spitting up.

I spent some time with my brother "Bean". I smiled when he kissed my hand. I like my furry brothers!
